The Primary Care Network supports this Alberta Health Services program with a part time Licensed Practical Nurse and part time Recreational Therapist.
Referrals to the program are made by a Home Care - Community Care Coordinator.
Adult Day Support - Program enhancement Black Diamond, Okotoks and High River
A Rehabilitation Aquatics Program adds to Alberta Health Services adult day support programs with a 6-8 week course of pool therapy which focuses on maximizing client capacity for work and daily living. The Primary Care Network supports the part time positions of a physiotherapist, administrative support and patient aide.
Patients can access the program through physician, home care, or health care professional referral.
