Palliative Care 

The Calgary Rural Primary Care Network wide program provides flexible care and support resources in the homes of palliative patients in this pilot project. The goals of the community palliative collaborative are to:

  • Enable palliative patients to remain in the community through flexible community funds when no other sources of funds are available.
  • Increase community capacity for palliative care (e.g. education and community development)
    Remove potential barriers to physicians’ involvement with palliative patients in their communities
  • Enhance linkages with specialists in palliative care

Physicians, home care, or health care professionals can present a case to the Palliative Care Team. The Primary Care Network works closely with the Regional Palliative Care Team to identify and assess eligible patients and approve expenses.
