The clinic supports older adult patients with complex conditions using a team-based approach to care. A thorough initial assessment is conducted to work through the complexities of patient condition(s).
The team includes:
- nurse case manager
- pharmacist
- social worker
- occupational therapist
The team works closely with referring physicians to develop recommendations for ongoing patient care.
The clinic is located in the High River Hospital/Annex and is linked with Alberta Health Services resources.
Patients are referred to the clinic by family physicians, home care nurses or other health care workers.
