Role Summary:
The Licensed Practical Nurse (LPN) will work within a multi-disciplinary team of physicians and allied health professionals to provide foot care and education on chronic disease management in various rural communities in Calgary Rural Primary Care Network’s (CRPCN).  This position’s aim is to support the goals and objectives of CRPCN.
Key Responsibilities:
  • Nursing tasks follow CRPCN prescribed policies, practices, routines and procedures
  • Foot care
  • Provide basic chronic disease management education
  • Seeks guidance from physicians and health care team members for more complex patient situations
  • Facilitation of group classes
  • Work with Electronic Medical Records (EMRs) to optimize patient care
  • Other duties as required within the LPN scope of practice
Educational Requirements:
  • Completion of an accredited Practical Nurse diploma program
  • Fully licensed and in good standing with regulatory body
  • Current BCLS certification for health care providers
  • Accredited Foot Care Certification
Experience and Skills Required:
  • 2 or more years of Licensed Practical Nursing experience, preferably in a primary care setting.
  • Experience with foot care.
  • Experience working within an inter-professional care team model is required.
  • Knowledge of and interest in primary health care, health promotion and prevention and chronic disease management would be an asset
  • Knowledge of EMR systems an asset

Working Conditions:
  • Clinical office environment
  • Reporting matrix structure of CRPCN management and representatives of physician clinics
  • Travel to additional CRPCN work sites will be required
  • Valid driver’s license & access to reliable vehicle required

Direct – Manager, Clinical Innovation; Indirect – Executive Director

The Calgary Rural PCN works together with communities, family doctors and other health team members to do whatever we can help you be as healthy as possible. We are working together to change the delivery of health care in our communities from reactive to preventative by providing services to patients that fit their needs through supports we have in our member clinics, such as nurses, dietitians and other health care professionals.
